Monitor system fixes

Nine monitor-layer issues fixed across three batches:

Batch 1 (M1, M2, M10):

  • M1: Stop infinite FR dispatch chain — follow-up scan no longer bypasses duplicate detection
  • M2: Circuit breaker for repeated remediation — per-project, per-fingerprint cap (5 attempts / 24h TTL)
  • M10: Default escalation severity corrected to warning + V23 schema migration for existing installs

Batch 2 (M3, M4, M5):

  • M3: Provider fallback chain — all errors now continue to next agent instead of aborting
  • M4: 10-minute dispatch cooldown gates only FR spawn (scan still records), includes error runs
  • M5: orphaned_task anomaly type registered in scanner

Batch 3 (M6, M7, M11):

  • M6: Type-aware action injection when FR returns only report_only for actionable anomalies
  • M7: Stable dedup fingerprints — canonicalize stuck-task types, exclude volatile fields
  • M11: Three-stage JSON extractor with string-boundary-aware backwards brace scan

All fixes verified via Playwright and dual cross-provider reviews (Claude + Gemini).
