Fix

Rate limit hammering in parallel runners: When Claude (or any provider) returned a 429 rate-limit error, the orchestrator loop in parallel workstream runners had no handler for the rate_limit action. It fell through to sleep(1000) and immediately retried — generating 211 failed invocations for a single task in minutes, burning through API rate limits.

Fix: added the same 60s backoff handler that already existed in loop.ts to orchestrator-loop.ts, which is the loop used by parallel runners.
