Move Google and Microsoft onto the OpenAPI plugin

Google and Microsoft services become catalog presets on the OpenAPI plugin, backed by spec-format adapters (Discovery conversion, Graph structural split). The dedicated provider plugins, add pages, and bulk picker are removed; each service is added and authed individually with only its own scopes. The grid groups provider services under a family umbrella.

Includes a one-shot migration that splits existing monolithic provider integrations into per-service integrations: a Postgres CLI for cloud (dry-run by default) and a boot-time data migration for selfhost, local, and Cloudflare hosts. Connections are cloned so no re-auth is needed.

Net diff is around -3,400 lines.
