[Foundation] Decouple catalog cardinality from team and concurrency limits

Outcome

Remove every functional dependency on the number 111. Treat the existing catalog as an extensible primitive library and derive concurrency from workflow width, runtime support, and explicit user policy.

Acceptance criteria

• Catalog schema accepts any non-empty unique valid primitive set.
• Intake, config, transaction, docs, tests, and generated artifacts contain no catalog-derived worker maximum.
• Installed role count and worker ceiling remain separate.
• Existing v0.1 catalog remains readable and migration-safe.
• High concurrency requires a digest-bound explicit acknowledgement.
