• Fix all_errors_fatal being ignored, as all_fatal was checked for instead. Both are now honored.
• errorPackageShort is now the last part of the package name, as documented, instead of the
  full package name with :: replaced with -.
• No longer warns about undef when warn or errorblank is called outside of a sub.
• Fix quoting of file and line in the errorblank message.
• Doc cleanup. Document the fatality checks and sync the synopsis and README with example.pl.
• Add tests for the error methods, fatality checks, and shared variables.
