executor-attach CLI tool

New executor-attach command to attach your terminal to any running executor-mcp managed process (GDB, minicom, Python REPL, etc.):

executor-attach          # auto-discover all processes system-wide
executor-attach <pid>    # attach by PID
executor-attach -l <log> # attach using log file

• Auto-discovery: scans /proc to find all executor-mcp instances and their children, reads CWDs to locate .executorlog/ directories across the entire system — works from any directory
• Press Ctrl-] to detach without killing the process

Improved prompt capture

• Stream reader now uses read(4096) instead of readline() to capture partial output (prompts like >>> , (gdb) ) that don't end with newline
• Log format distinguishes complete lines (:) from partial output (>) for better attach experience
