Fixed ctrl+o ("Toggle tool output") not expanding tool output for tool rows grouped into a tool shelf (the running/most-recent turn): the shelf now propagates output expansion to its nested rows and reveals them when expanded, instead of flashing and reverting (#134).
/model <search> now surfaces pricing refresh failed instead of silently swallowing the pricing refresh error (was fired fire-and-forget with no handling) (#133).
