Updated migration guidance

Current setup and upgrade instructions, including rc.52/Pi registry incompatibility, shared Git worktrees, and copied canonical agent files, are in Migrating to 1.0.0. The README now includes architecture and upgrade diagrams. These documentation corrections do not change the v1.0.0 tag or its installed payload.
