Fixed

• Added a complete Codex account transport fallback: native Responses
  WebSocket first, immediate HTTPS SSE fallback for HTTP 403/426, bounded
  retries for other upgrade and transient failures, sticky HTTPS selection per
  session, and safe turn replay when a WebSocket stream is interrupted.
• Made both Codex transports honor environment proxies, NO_PROXY, HTTP
  CONNECT, SOCKS5, macOS system and PAC proxy settings, platform root
  certificates, and explicit CA bundles. Allowlisted Cloudflare infrastructure
  cookies are shared across transports without exposing unrelated cookies.
• Recovered HTTP 401 responses by first reloading credentials rotated by Codex
  CLI and then refreshing OAuth once when necessary. Account usage limits stay
  terminal and do not trigger amplified retries.
