v0.1.0 Format for entries/_template.md, updated README

Simplified template for recording entries in the repository.

Features of entries/_template.md are:

• Simple title, with short comment to describe it
• Previous entry below the title and Next Entry commented out.
• The date is to be added for each entry below the Navigation section above
• In addition to date other aspects like repo, PR, Issues or commits can be listed.
• List of 5 sections that can be edited for convenience with short descriptions
• The Continues From or Continued In for referencing across entries
• The tags section which can be edited on adding entries for reference.

The above should be helpful for getting started for effectively recording the progress.
