Add creator image fallback state

Summary

Add a fallback UI state for creator images when the primary image is missing or fails to load.

Acceptance Criteria

• fallback renders consistently across creator card and profile contexts
• image load failures switch cleanly to the fallback state
• fallback styling matches the product UI
