Add selection accumulator extension

Introduce a new VS Code extension that accumulates text selections for batch copy/cut. extension.js implements an AccumulatorProvider TreeView with accessible labels, stores marked selections (with preview and document range), and provides commands: accumulator.mark, copy, cut, clear, moveUp, moveDown, deleteItem, and focus. Copy/Cut write combined text to the clipboard (using configurable separators) and cutting removes ranges only from the active document. package.json registers activation events, commands, keybindings, Explorer view, and configuration options (selectionAccumulator.maxItems, separator, customSeparator).
