Two fixes for MCP tools.

馃悰 Bug fixes

  • Stdio MCP tools stopped working after their first use. After the first tool session, the agent could not talk to the server anymore, and every later request failed with "exec: Stdout already set". The client reused a subprocess handle that can only be started once, so any reconnect died instantly. Each connection now starts a fresh subprocess, and stdio servers keep working across reconnects and new sessions.
  • Agents silently lost tools when pointing at a missing MCP server. If an agent referenced an MCP server that no longer existed, the server was skipped with no log line anywhere, and the agent came up without those tools. The skip is still tolerant, but it now logs a warning naming the agent and the missing server, so a stale reference is no longer invisible.
