fix: OAuth model restriction + ConversationPhase theme for light mode

OAuth tokens (sk-ant-oat) can only use Haiku — restored the model
restriction that was overwritten by the PKCE commit. Also migrated
ConversationPhase from hardcoded dark-mode classes to CSS variable
theme system so it's readable in light mode (default for new users).
