chore: lint pass — remove dead code, fix stale models, harden config

- Delete AISetup.jsx (replaced by SimpleAISetup everywhere)
- Replace stale gpt-4o with gpt-4.1-mini in env-status route
- Default model fallbacks use Haiku (safe for OAuth) instead of Sonnet
- Wrap bare chmodSync() calls in try-catch (config.js)
- Bundle is 7KB smaller (71 modules, down from 72)
