Bug fix retrospective and commit gate improvements.

What's new

  • Bug fix retrospective — commit gate now requires detection gap analysis, concrete prevention artifact, and pattern scan before allowing bug fix commits
  • once: true on SessionStart — prevents redundant discovery runs
  • Task completion enforcement — Stop hook continues to next slice instead of handing off when planned work remains
  • Cleanup — removed duplicate hook definitions from SKILL.md frontmatter; hooks.json is the single source of truth

Fixed

  • Marketplace source pointed to dead /tmp path; updated to GitHub repo
