Include agent metadata in instrumentation middleware — The instrumentation middleware now merges agent-defined metadata (from the agent's metadata method) into execution records. Agent metadata serves as a base layer, with middleware metadata overlaid on top. This makes custom agent metadata visible on the dashboard immediately, without requiring a separate process_response override
