Mutation lifecycle client scaffold (Phase 2)

  • Add mutation-lifecycle-contract.md — handle discipline + dual-stack ref-first rules (#66)
  • QuickCreate append: qualify counts as execution.campaign_count / execution.adset_count (#130)
  • QuickCreate ↔ Quick Copy path ownership table in meta-copy contract
  • Recovery matrix: ref-first rows when hosted durable handles are advertised

Pairs with smartads PR #135 (cd1e7de7) for hosted field-placement repairs.

Recommended after: Meta deploy of #135. Compatibility ledger remains 0.7.55..0.7.57 until a separate Meta #99 bump.
