Support model prompts directly in models.json

Keep custom model transport, capabilities, and behavioral instructions together by configuring model-specific system prompts directly in models.json, while retaining the existing settings.json surface.

What changed

  • Custom model prompts. Custom models[] entries can set either systemPrompt or appendSystemPrompt alongside model metadata.
  • Built-in model overrides. modelOverrides entries can apply the same replacement or append behavior to built-in models.
  • Loud source conflicts. Configuring prompt behavior for the same canonical model in both models.json and settings.json fails explicitly instead of applying hidden precedence.
  • Complete lifecycle support. Prompt metadata applies during startup, model switching and cycling, session restore, and /reload; changed or removed models-file prompts refresh without restarting.
  • Validation and documentation. Empty, malformed, and replacement-plus-append declarations are rejected, with examples and rules documented across the README and configuration guides.
