GPT-5.6 max reasoning effort

Add a distinct, model-aware max thinking tier while preserving xhigh behavior across existing providers.

What changed

  • Model-aware max support. GPT-5.6 aliases and Sol, Terra, and Luna variants expose native max, with safe fallback to xhigh or high on unsupported models.
  • End-to-end configuration. CLI, model shorthand, settings, RPC, SDK/extensions, dispatch arbitration, subagents, session persistence, TUI, dashboard, and Telegram all understand the expanded thinking scale.
  • Provider-safe requests. OpenAI Responses, Codex/Responses Lite, Azure Responses, and compatible APIs carry max without changing established Claude, Kimi, Qwen, Google, or Bedrock mappings.
  • Model-aware selectors. TUI and dashboard controls expose only supported levels and keep model, thinking level, and available levels synchronized after model changes.
  • Clear ultra semantics. Documentation distinguishes native provider effort max from Codex ultra, which adds client-side multi-agent orchestration and is not sent as a raw effort.
  • Safer theme upgrades. Invalid custom themes now report validation errors before falling back to the built-in dark theme.

Implemented in PR 481.
