セキュリティ修正

GitHub Actions の actions/checkout と actions/setup-node をリリース済みコミットSHAへ固定。
pull request用CIで checkout の認証情報をワークスペースへ永続化しないよう設定。
上記2点を検査する回帰テストを追加。

検証

npm run typecheck
npm test（73 tests passed）
node dist/src/cli.js check --json（PASS）
standalone Skill bundle の --version が v1.0.3 と一致。
