feat: release v8.99.3 — fix(codex): stop false-positive loop kills when bursty file_writes are 80-char-truncated to the same prefix — LoopDetector now keys on raw paths with a 15-write threshold, while non-file_write events still trip on 8/10 identical untruncated keys (#125)
