Fixes fresh-machine Claude/Codex plugin installation paths and Hephaestus naming.\n\n- Renames plugin selector/display from agentlas-meta-agent to hephaestus / Hephaestus.\n- Corrects Codex docs: install with OS-terminal codex plugin ...; inside Codex use /plugins to browse installed plugins.\n- Adds a macOS Command Line Tools preflight for git/xcode-select clone failures.\n- Ships lowercase /hephaestus command files and executable runner bits so ontology GUI startup works from plugin caches.\n- Validated Claude install, Codex install, package verification, public safety, and ontology runtime tests.
