Two new fields on the Puzzle struct for better provenance tracking.

Highlights

• source_url field (#8): Links back to the original source where each puzzle was published. Now you can trace any puzzle to its origin.
• start_date field (#7): When the puzzle or bounty was first created. Useful for tracking how long puzzles have been standing unsolved.
