* fix(config): default tmp envs
Set WORKSPACE_DIR default to /tmp and ensure codex HOME env
is set for subprocesses. Update gateway fake to satisfy
volume/log interface additions.
* fix(daemon): codex home fallback
Use codex HOME fallback for config, skills, and mapping store
paths when HOME is unset. Add coverage for fallback behavior
and update the workspace default assertion message.
* fix(e2e): stub token counting server
