v0.18.3 — Leader correctness (adversarial-audit fixes)

Correctness fixes for the --mode tmux production leader, from two fresh adversarial-audit passes (each verified against the actual code paths).

Fixed

  • Consistent user-story coverage — the story list and completion-coverage count are now derived only from ### US-NNN: headings. A US id mentioned in prose/dependencies ("builds on US-009", "Depends on: US-001") no longer inflates them, which had stopped some per-US campaigns from recognizing completion.
  • Valid-JSON completion signals — a Worker/Verifier that exits having written a truncated/empty signal is no longer accepted as a finished result.
  • Verifier exit routing — a Verifier that exits without a verdict is relaunched as a verifier (correct model/prompt), not mis-restarted as a worker.
  • No-commit repositories — dirty-work detection now functions in a brand-new repo with no commits yet (a Worker that staged but never committed could previously go undetected).
  • No stale verdicts — the final verification no longer reads a previous iteration's verdict on recovery/finalize iterations.
