v2.0.4 - Agent-first defaults + legacy cleanup

Major cleanup release that removes legacy compatibility code, aligns with the official Claude Code Skills spec, and restructures the codebase for clarity.

✨ Highlights

• Agent-first by default: All commands are now non-interactive by default
  • Use --tui for interactive mode (replaces -y/--yes)
  • sync uses transclusion by default (--direct for inline embedding)
  • sync creates AGENTS.md if missing instead of erroring
• Session hook fix: Fixed PATH corruption bug - hook now uses ${PATH:-fallback} to prevent broken shells
• Official spec alignment: SKILL.md frontmatter now strictly follows Claude Code Skills spec

🗑️ Legacy Removal

• Removed session-hook command (replaced by bash script)
• Removed skill-prompt command (use tool-description)
• Removed non-spec frontmatter fields: when_to_use, disable-model-invocation, snake_case variants

🏗️ Codebase Restructure

Source reorganized from flat src/utils/ to domain-specific directories:

• src/skill/ - Core skill logic (discovery, validation, frontmatter)
• src/agent/ - Agent platform integration (plugins, agents-md)
• src/config/ - Configuration and paths
• src/marketplace/ - GitHub integration
• src/telemetry/ - Usage tracking

Tests restructured to mirror src/ layout. All 147 tests pass.
