HITL Approval Flow — --approve flag adds a manual review checkpoint before saving generated tests.

HTML Replay — --list-html-replays and --replay-html-analysis <run_id> for debugging and auditing past HTML analysis snapshots.

Modular Architecture — Runtime split into qa_automation.py, qa_config.py, qa_runtime.py, and qa_workflow.py. No breaking changes.

Update pinned images to ghcr.io/aiqualitylab/ai-natural-language-tests:v4.2.0.
