AgentMFA 0.1.0

This is the first release of AgentMFA, including the mfa CLI and
MacOS desktop app, running against local and remote AgentMFA instances.

Supported connections include:

• Postgres
• SSH
• MCPs (including OAuth)
• HTTP APIs
• WebSockets
