docs: rewrite for unified ralph client with managed and BYOK modes

- Single product, single binary — no more separate Ralph/Ralphy pages
- Two modes section: managed (default) and BYOK (--byok flag)
- Setup and usage examples for both modes
- Fix JS to fetch releases from ralph-releases repo
