Stop the Unix focus-hook replace loop

Fixes

  • Hold the Unix ensure lock until a newly spawned sidebar stamps its identity token, preventing left-dock focus events from repeatedly closing and replacing the pane. Fixes #29.
  • Add regression coverage for the startup-token probe and document the hook invariant.
