  • fix(hooks): stop instruction-ledger self-enrollment — the ledger extracted every markdown bullet including lines inside code fences, so pasting an agent completion report enrolled its own summary as new pending work and Stop could never be satisfied (c452f9df)
  • fix(hooks): stop instruction-ledger and review-fix-ladder gates failing open — jq -e over a generator emitted nothing at zero pending nodes and exited 4, and a bare sb_rfl_compliance_stop under set -e plus trap 'exit 0' ERR made a deny gate return ALLOW (8f570b37)
  • fix(hooks): add a real claude arm to sb_leanctx_hooks_present() — the hooks-wiring check silently passed on every host except cursor and codex (c452f9df)
