First release. A Windows driver for Generic USB Display devices, written so a per-game modeline reaches a 15 kHz CRT.

The display appears as an ordinary monitor and the desktop extends onto it.
Modelines can be added while it is running — the driver picks them up within
about two seconds and re-enumerates, no replug. A Switchres backend
(gud-backend branch of alphanu1/switchres) generates a timing per game and
drives it end to end.

Read INSTALL.md before installing. The driver is self-signed, so installing
means trusting a certificate as a root — that is a real decision and the guide
explains exactly what it means. Windows Defender may also flag the driver as
Trojan:Win32/Bearfoos.A!ml; it is a false positive, it has been reported to
Microsoft, and the guide covers what to do.

Tested on one device (blitsCRT_Mister) and one machine, Windows 10 19045.
