Worker execution-classification reconciliation hotfix.

  • Binds formal Worker report execution classification into ACK-ready staging handles.
  • Adds paused-safe-point reconciliation for already archived misclassifications.
  • Adds real-incident regression coverage and preserves Pack/task/history identity boundaries.

Evidence boundary: repository runtime accounting only; this release does not convert a failed Local Verification into PASS or claim Codex app-server process cleanup is fixed.
