mtqg v1.1.0

Removes the --at feature (the CLI flag and the MCP path/line
parameters that attached a record to a file and line). Seeing it used
in mtqg-vscode's UI showed that binding a record to a code location
was not worth the complexity, since code moves. The format version
stays 1: at was always an optional field, so old journal.jsonl lines
that still carry it keep reading fine, just without that field
appearing anywhere in mtqg's own output any more.

Also adds a freshness marker so mtqg upgrade keeps .mtqg/SCHEMA.md
in sync with docs/reference/schema.md's wording even when the format
version itself does not change (as with the --at removal above).
.mtqg/version is untouched.
