v1.45.2 -- Credential Request Fixes

Bug Fixes

• Credential mode persistence -- Non-admin credential mode selections no longer revert on page refresh. Mode is saved to DB immediately.
• Read tool shortcut -- Read tools with existing team connections no longer create unnecessary requests; mode is set directly.
• Admin-only approval -- Only platform admins can approve credential requests. Agent owners can no longer self-approve.
• Approve sets mode -- approveToolRequest() now sets credential mode to team (previously lost the user preference).
• Dashboard buttons -- Credential Requests tab now shows Approve+Deny for write tools, Configure+Deny for read tools.
• Pending indicator -- Agent tools page shows amber Pending approval next to credential dropdown for tools with pending requests.
• Hidden write button -- Can make changes button hidden for non-admins when write access is not active.
