Improves first-run behavior and documentation. The CLI now explains that capture starts on subsequent requests, warns when no query has matched, clarifies that the live reader waits until Ctrl+C, and gives an all-query verification command. Adds automated PHP linting on PHP 7.4 and 8.4.

Query Origin Tracer is now a standard WordPress plugin with an activation-managed early MU bootstrap. This release adds clean activation, deactivation, uninstall, and automatic migration from the legacy MU-only v1 package. The attached ZIP can be installed directly through WordPress Admin or WP-CLI.
