Durable session fix

  • Prevents forced account switches while Codex or Desktop helpers can still write the live credential store.
  • Stops inactive-account quota checks, activation, and usage-window automation from rotating saved refresh tokens.
  • Requires add-account and re-login flows to observe a genuinely changed credential and verify it with OpenAI before reporting success.
  • Pauses automatic switching and quota work during login, while safely closing and restoring Desktop around the transaction.
  • Preserves account snapshots and does not require a fresh login merely because the app was updated.

Accounts whose refresh tokens were already revoked before this update may need one final sign-in. Existing roster data and saved snapshots remain intact.
