Project edit dialog does not persist name or icon changes

Description

When editing a project from the OpenCode web sidebar, changing the project name or icon does not reliably persist to the sidebar project item after clicking Save.

For local/global projects, the edit dialog saves the updated name/icon metadata, but the sidebar can continue showing the previous fallback initials/name because the saved local project metadata is not merged back into the displayed project list.

There is also a preview issue where the Avatar component can keep showing fallback initials instead of reacting when an uploaded image src is set.

Steps to reproduce

1. Run opencode web.
2. Open the web interface.
3. Right-click a project icon in the left sidebar.
4. Select Edit.
5. Change the project name or upload an image in the Icon field.
6. Click Save.
7. Observe that the sidebar project item can continue showing the old name/icon or fallback initials.

This issue might be a duplicate of existing issues. Please check:

i went through #24197, #22360, #17801, and also the related #14465 / pr #17807.

this one lines up closest with #24197, especially the bit about non-git projects falling back to the global project. the issue here is basically in how things are displayed: the edit dialog already saves local/global project metadata, but the sidebar never merges childStore.projectMeta back in when server metadata is missing or global. so after hitting save, it looks like nothing happened for things like name or icon.

it’s not the same as #22360, which is more about localStorage limits and icons getting evicted when they get too big.

also different from #17801 / pr #17807. that one fixed backend persistence (icon.override not surviving restarts). this change doesn’t touch backend or schema at all, it’s purely fixing how the frontend sidebar builds and updates the project view, plus making Avatar react properly to src changes.

so yeah, i wouldn’t call #24744 a duplicate. it’s related to #24197 and might incidentally fix that fallback case, but it also handles project name updates and comes from a different fix path.
