Description
When editing a project from the OpenCode web sidebar, changing the project name or icon does not reliably persist to the sidebar project item after clicking Save.
For local/global projects, the edit dialog saves the updated name/icon metadata, but the sidebar can continue showing the previous fallback initials/name because the saved local project metadata is not merged back into the displayed project list.
There is also a preview issue where the Avatar component can keep showing fallback initials instead of reacting when an uploaded image src is set.

Steps to reproduce
- Run
opencode web.
- Open the web interface.
- Right-click a project icon in the left sidebar.
- Select Edit.
- Change the project name or upload an image in the Icon field.
- Click Save.
- Observe that the sidebar project item can continue showing the old name/icon or fallback initials.
