Apply Patch / Edit permission view only shows the first file

Description

When the apply patch tool requests permissions to change multiple files at once, the user can only see the first file and has no way to see the other files.

For example in the image below, the user can only see the edit propsed for the first file, not the second.

Steps to reproduce

1. Use a model that prefers the 'apply_patch' tool.
2. Get it to suggest an edit to multiple files.
3. Make sure you're not on auto-approve mode and instead it requests permission.
4. You can only view the first file.

It is working fine

No it is not! You need to make sure the agent is using 'preparing patch' tool, not the 'edit' tool. And that agent is making changes to multiple files at once.

Are you using openai models through chatgpt subscription? They always use 'apply_patch' over 'edit', which is where this issue happens.

I tested with GLM 5.2 via Opencode zen, and it doesn't happen. Please use an OpenAI model through ChatGPT subscription and you'd see the issue.

I can confirm that it is broken at least for OpenAI models, tested with 5.5 and 5.6.
A very annoying issue that should be fixed.
