…on services Switch from Tool.define with static Session.*/Question.* calls to Tool.defineEffect yielding Session.Service and Question.Service. All session and question operations are now direct yield* calls on the service instances.

Mechanical conversion of the
plantool's execute method from async/await toEffect.gen+Effect.promise, withEffect.runPromiseat the boundary. No behavioral change — precursor to eventually making tool execute return Effect natively.
