fix(opencode): recover expired websocket auth

Summary

• recover ChatGPT OAuth WebSocket connection failures through HTTP so authorization responses remain visible
• refresh tokens five minutes before expiry and force one refresh-and-retry after a 401
• surface permanently rejected refresh tokens as non-retryable authentication errors with login guidance
