fix(task-packets): sharpen focused bug-fix packets

Highlights:

keeps bug-fix packets anchored to explicit changed files more reliably
filters legacy-model and peripheral utility-script paths out of focused key-file selection when changed files are known
keeps sparse-graph dependency hubs biased toward shared dependencies instead of false-positive utility files
updates the README and quickstart site to reflect the tighter bug-fix packet behavior
