[BUG] Max plan defaults to Sonnet despite 5% usage (should use Opus 4.5 up to 20%) #16875

What's Wrong?

aI'm subscribed to the Max plan, but Claude Code is defaulting to Sonnet 4.5 instead of Opus 4.5.

According to the documentation, Max plan users should see:
"Opus 4.5 for up to 20% of usage limits, then use Sonnet"

However, when I run /usage, it shows I'm only at 5% usage (just reset today), but the default model is still showing as Sonnet instead of Opus.

This appears to be similar to issue #6831, but that was filed 5 months ago for Opus 4.1. This is happening now with Opus 4.5 / Sonnet 4.5.

What Should Happen?

aSince I'm on the Max plan and only at 5% usage, the default model should be Opus 4.5.

The model selector should display:
"Opus 4.5 for up to 20% of usage limits, then use Sonnet"

Opus 4.5 should be the default model until I reach 20% of my weekly usage limit.

Steps to Reproduce

a1. Have an active Max plan subscription
2. Open Claude Code CLI (version 2.1.1)
3. 3. Run /usage command to check current usage
4. 4. Observe that usage is at 5% (weekly limit just reset)
5. 5. Run /model command to check available models
6. 6. Observe that the default model is showing as "Sonnet 4.5 for daily use" instead of "Opus 4.5 for up to 20% of usage limits, then use Sonnet"
