VS Code extension commands not working - "command not found" errors

What's Wrong?

Description:
After installing Claude Code CLI and VS Code extension, all extension commands fail with "command not found" errors.

Environment:

• OS: Windows
• VS Code: 1.109.5
• Claude Code CLI: 2.1.55
• Extension: anthropic.claude-code v2.1.55

Error messages:

• command 'claude-vscode.editor.openLast' not found
• command 'claude-vscode.terminal.open' not found
• command 'claude-vscode.editor.open' not found

What works:

• Running claude directly in terminal works fine

What I tried:

• Reinstalled Node.js
• Reinstalled Claude CLI
• Reinstalled VS Code extension
• Cleared extension cache
• Added code command to PATH
• Ran claude upgrade

None of these fixed the extension button issue.

It was working fine until yesterday, but today my VS Code extension stopped working. I downgraded to 2.1.49 and now it works.
