Cannot remove pasted photo after size limit error - session gets stuck

Description

When pasting a large photo into the Claude Code chat (VSCode extension), the user gets an error message saying to reduce the size before pasting. However, after this error:

1. The photo that was already added cannot be removed
2. The session becomes stuck - the user cannot continue working
3. The only workaround is to start a new session (/clear)

Steps to Reproduce

1. Open Claude Code in VSCode
2. Paste a large image (e.g. >5MB) into the chat input
3. Observe the "reduce the size" error message
4. Try to remove the pasted photo — there is no way to do so
5. The session is now stuck

Expected Behavior

• There should be a way to remove/dismiss a pasted photo (e.g. an X button on the thumbnail)
• After a size limit error, the oversized photo should be automatically removed or the user should be able to dismiss it
• The session should not get stuck

Actual Behavior

• No option to remove the pasted photo
• Session becomes unusable after the error
• User is forced to clear the entire session and lose context
