What's Wrong?

When Claude Code Chat wants to edit a file, it asks for permission to edit. I am offered 3 choices, 1. Yes 2. Yes, allow all edits this session 3. No. Clicking 1 or 2 makes no difference, I am repeatedly asked for permission to edit the same file. 18 times I was prompted for the same file before I just stopped using the chat.

What Should Happen?

When option 2. is selected, edits should be allowed for the entire session. I should not be prompted to allow again.

Error Messages/Logs

No errors were presented, just the same behavior repeated.

Steps to Reproduce

1. Open a project in VS Code
2. Ask Claude Code Chat to create a file test.md in .claude folder
3. Ask Claude Code to update that file in a way that will cause it to make more than one edit
4. When prompted to allow edits, choose option 2. Yes, allow all edits for this session
5. Witness the same prompt repeated for eternity.

This is not a repeat. I have already looked. This is for VS Code extension, not terminal, not on a Mac, it is not for Yes/No only options.

It's possible the root cause is the same as these other reported issues, but the environment is different. I'm using the vscode extension and getting the same behavior as those using the terminal in PowerShell, Mac, or Linux.

Experiencing the same issue.

I have "Edit" and "Write" (unmasked, no path restriction) in both my global user settings (~/.claude/settings.json) and my project-local settings (.claude/settings.local.json). Despite this, the VSCode extension prompts me every single time it wants to edit a file — even for files under .claude/skills/ which I edit frequently.

Selecting "Yes, allow all edits this session" (option 2) doesn't persist either — I get prompted again on the next edit within the same session.

Environment:

• Windows 11 Pro
• VSCode extension: anthropic.claude-code v2.1.78 (win32-x64)
• Claude Code CLI: v2.1.49

Both plain Edit and Write rules should be blanket allows with no path restriction, yet the prompt fires every time. The CLI does not have this issue — only the VSCode extension.

It was working fine with the previous VScode extension release if I'm not mistaken. And it seems to specifically happen when I ask Claude to update files under the .claude/skills folder

Same experience here. Using RPI to create session files under $HOME/.claude/thoughts/shared/sessions, plan files and research files it asks for every edit, even if Edit Automatically is set. Even if those paths are in additional files, even if you have permisions edit/write with those paths.
