What's Wrong?

Describe the bug
/doctor reports a failing MCP server named claude_design. Running /mcp shows:

Failed to reconnect to claude_design: HTTP 404 at https://api.anthropic.com/v1/design/mcp

Steps to reproduce

1. Run /doctor in Claude Code
2. Run /mcp to inspect

Expected behavior
Either the endpoint exists and the server connects, or claude_design is not included as a built-in server if the endpoint is not available.

Actual behavior
HTTP 404 from https://api.anthropic.com/v1/design/mcp. The server cannot be removed via /mcp UI or settings files — it does not appear in settings.json or settings.local.json, suggesting it is hardcoded in Claude Code itself.
