Bug Description
I was working on a VPN client, nothing violating Claude Fable 5's rules, wrongly flagged by safeguards and switched to Opus 4.8.

Same class of false positive, with request IDs for tracing.

Context: Reviewing security hardening in my own local Electron app (permission allow/deny parsing, single-use spawn authorization tickets). Pasting a code reviewer's findings inline — text discussing "fail-open paths," "upgrade permissions," "defeating the ticket's single-use intent" — got the message flagged by Fable 5's safeguards. Subsequent retries that re-included the pasted review also flagged, including one message that was otherwise only a few words.

Environment: darwin, Claude Code 2.1.220, model claude-fable-5[1m].

Workaround that recovered the session: switchModelsOnFlag: true in settings.json (auto-retry on Opus), plus moving the pasted review into a file referenced by path instead of inline message text.

Legitimate defensive security review of one's own codebase seems to be a recurring trigger surface — the flagged content was a code review recommending stricter permission handling.
