Premise+hypothesis chunking with cartesian-product batch scoring
replaces hypothesis-only chunking that truncated long RAG contexts.
Prometheus output now emits HELP/TYPE headers and le="+Inf" buckets.
HTTP request metrics middleware added. 22 new tests, all 801 passing.
